Start before the appointment
An unpaid claim often begins with an administrative detail that could have been identified earlier. A changed insurance plan, an incorrect member number or an unconfirmed referral can create work after the visit. Build a pre-visit checklist that assigns responsibility for demographic review, eligibility and any payer-specific requirements.
Benefits information is an estimate of coverage, not a promise of payment. Record the source and date of the check so staff know what was verified and when.
Separate submission from acceptance
Sending a claim does not mean the payer has accepted it for processing. Keep clearinghouse rejections separate from payer denials. A rejected claim may need a corrected field, while a denied claim may require documentation, an appeal or clarification of a benefit rule.
Give each unresolved item an owner, a next action and a follow-up date. Avoid a single status such as pending that hides the reason work has stopped.
Reconcile payments and outstanding work
Payment posting should connect the remittance to the claim and the deposit. Review adjustments under the practice’s approved policies and escalate unexpected reductions rather than treating them as routine write-offs.
A useful weekly review includes unsubmitted charges, rejected claims, outstanding payer requests and aging balances. A monthly report should explain open issues as well as collections.
Adapt the workflow to your specialty
Mental health, primary care, dental, endocrinology, home healthcare and remote patient monitoring have different administrative needs. Confirm the billing workflow and documentation responsibilities for the services your practice actually delivers.
Care Tribune can help organize claim submission, posting and follow-up within an agreed scope. Start with a review of your software, payer mix and the backlog you want to address.
